Family Health Plus

Health Insurance for Uninsured New Yorkers

Family Health Plus is a state funded health insurance program for adults between the ages of 19 and 64. Participants must not have health insurance and their incomes must be too high to qualify for Medicaid. Single adults, couples without children, and parents with limited income who are residents of New York State and are United States citizens or fall under one of many immigration categories may qualify for Family Health Plus.

Is My Family, or Am I, Eligible for Family Health Plus?

Your eligibility depends on your income and the size of your family. For the most current eligibility information, please see the Family Health Plus section of the New York State Web site.

Will I Have to Pay a Premium?

There are no premiums for Family Health Plus but you may be asked to pay a copay for some services.

What Benefits Does Family Health Plus Provide?

Family Health Plus provides the following benefits:

  • physician services
  • inpatient and outpatient hospital care
  • prescription drugs and smoking cessation products
  • lab tests and x-rays
  • vision, speech and hearing services
  • rehabilitative services (some limits apply)
  • durable medical equipment
  • emergency room and emergency ambulance services
  • behavioral health and chemical dependence services (which includes drug, alcohol and mental health treatment - some limits apply)
  • diabetic supplies and equipment
  • hospice care
  • radiation therapy, chemotherapy and hemodialysis
  • dental services (if offered by the health plan)

Who Will Be Our, or My, Health Care Providers?

You must select a managed care plan when you apply for Family Health Plus. You should be able to pick a plan that includes your current physicians.

How Do I Apply for Family Health Plus?

The application process includes a personal interview. You must meet with an enrollment facilitator or staff at your local department of social services office:
